TMS and MeRT for Veterans with PTSD

Many veterans have done everything they were told to do for PTSD. They’ve tried therapy, tried medication, waited it out. And still the nightmares, the hypervigilance, and the short fuse remain. If that sounds familiar, you may be looking for something different.

Brain-based treatments like TMS and MeRT are getting more attention from veterans, and for good reason. Here’s an honest look at what they are, what the evidence shows, and how coverage works with Tricare.

Why PTSD in Veterans Can Be Hard to Treat

PTSD isn’t a weakness or a character flaw. It’s a change in how the brain processes threat. Combat, training accidents, blast exposure and repeated high-stress deployments can leave the brain’s alarm system stuck in the “on” position.

For many veterans, PTSD also comes with:

  • Depression, which often shows up alongside PTSD
  • Traumatic brain injury (TBI) from blasts or head impacts
  • Sleep problems, which make every other symptom worse

This overlap is one reason a single approach doesn’t work for everyone.

What Is TMS, and Can It Help PTSD?

Transcranial magnetic stimulation (TMS) uses magnetic pulses to stimulate specific brain regions. It’s non-invasive and drug-free, and you’re awake and alert during each session.

TMS is FDA-cleared for depression and has been studied for PTSD, including in VA research. The results are promising, but standard TMS isn’t FDA-cleared specifically for PTSD. That matters for insurance. For many veterans, TMS that treats their depression also eases the weight PTSD puts on daily life.

What is MeRT?

Magnetic e-Resonance Therapy (MeRT) builds on TMS technology with one key difference: personalization.

  • Before treatment, you get a qEEG brain map, which records your brain’s electrical activity, along with an EKG.
  • Your care team uses that data to set the stimulation to your brain’s own rhythms. Every patient doesn’t get the same protocol.
  • Your protocol is adjusted as your brain responds.

In 2026, MeRT received FDA clearance for the treatment of PTSD. That’s a major milestone for a therapy that has been used with veterans and special operations communities for years.

What Does Tricare Cover?

This is where many clinics blur the lines, so here’s the straight answer:

Treatment Tricare coverage
TMS for major depression (adults 18+, after two failed antidepressants, preauthorized) Covered. Brainwave is in-network with Tricare West
TMS when PTSD is the only or primary diagnosis Not covered
MeRT for PTSD, TBI or depression Not covered. TriWest considers MeRT an emerging technology
  • If depression is your primary diagnosis, TMS may be covered by Tricare, even if you also live with PTSD.
  • If MeRT is the better fit, it’s available as a self-pay option at Brainwave, with 20% off for veterans, service members, and first responders.

We’ll lay out both paths at your consultation, with real costs, before you commit to anything.

What Treatment Looks Like at Brainwave

  1. An initial consultation with our provider to talk through your history, goals and coverage. Tricare Prime members need a PCM referral for this visit. Tricare Select members can book directly.
  2. A brain map (qEEG) if you’re exploring MeRT. It’s non-invasive and uses no radiation.
  3. Short weekday sessions of about 30 minutes over roughly six weeks. Most people drive themselves to and from appointments.
  4. Progress check-ins throughout, so your care adjusts as you do.
